What & How to Report
How to Report
Reports of suspected abuse or neglect can be made to either Child Protective
Services (In Maricopa County, 530-1800; Statewide, 1-800-541-5781) or to the local
law enforcement agency. The mandatory reporting law stipulates that mandated sources
provide written reports in addition to reports made by telephone. Reports should
always be made immediately. Reports can be made 24 hours per day, 7 days a week.
What to Report
Any form of non-accidental injury; physical abuse; sexual assault, molestation
or exploitation; inadequate supervision; improper/inadequate medical care or physical
neglect that constitutes a potential health hazard; and circumstances suggestive
of emotional abuse or neglect should be reported.
First hand information is desirable, although not required. Whenever possible,
the source should provide specific and timely information--especially as it regards
the nature and extent of injuries resulting from mistreatment. Additionally, the
source must be able to provide certain identifying information regarding the children
and their families to allow for follow-up contact. Other information is also very
helpful. This information might include the children's names, approximate ages,
names of schools, parents' place of employment, etc. The more current and detailed
the information, the less likely a serious referral or family in need will be
under served.
